ESR recommends using q4replay to play back Quake 4 demos. You can download q4replay on the following page:

If you want to try playing back the demo manually, place the demo in your Quake 4 folder under the following path:
Quake 4\baseq4\demos\
The most efficient format for Q4 demos are netdemos (extension=.netdemo), to playback netdemos, run Quake 4, open the console (ctrl, alt, tilde key by default) and type in playnetdemo <demoname>.
If you have problems playing back the demo you may not have all the required maps. We recommend using q4replay to avoid this problem.
