Got multiple monitors connected?

if your refresh rate is 120hz and you're getting 115-120fps, then there could be some desync occurring. another issue could be that quake champions is falsely reporting fps performance during these tearing moments, and your fps could be dropping substantially for some reason.

make sure you have updated drivers, lower your in-game graphics settings as much as possible, and try playing at a reduced fps if possible (60?) to see if the behavior can be reproduced with more consistent fps.

You'll pretty much always get screen tearing at some point no matter what game you play unless you're using vsync or gsync/activesync, or fast sync, but fast sync needs much higher fps than refresh rate.
