125 is possible - it defaulted to lower the value to avoid performance problems due to more players causing issues for those with lower spec pcs

You can change the fps limit when you create a server by making a custom .cfg file in the "Mods" folder in q4max. For example, you want to play CTF:
1) create a .txt file in "Quake 4\q4max\modes" rename it to whatever you want but add ".cfg" as a file extension (CTF125fps.cfg).
2) Copy paste these values inside:
timelimit 20
caplimit 0
teamdamage 0
weaponrespawn 5
gametype 3
tickrate 125

As you might have guessed, the tickrate is the fps limiter. I for example have a 144 hz monitor and like to use "tickrate 144" when I create a server.

3). Check to see how it runs. Double click on "Start Q4Max.bat". Click on Multiplayer -> Create Server -> on the right side change the gametype to "CTF125fps".

If you want to learn more about Mods, go to "Quake 4\q4max\docs\Modes.htm"

I don't know exactly how to create a dedicated server with a custom mod, but maybe someone will help you on that one :)

Exactly as you describe - upload your ctf125fps.cfg to the server, set si_mode to "ctf125fps" and run as normal (check si_modes for the allowed modes is voting)

has this been tested for ctf servers? regardless if it's unstable for others I can change it. and can I get away with changing it on a rented Q4 server or am I going to need to rent a VPS and set it up manually?

iirc amazon is $30. you just need the key so you can buy it off Q4 and install it manually (most torrents are the original iso and a crack patch)

you can get it for 5 bucks on amazon. also steam's version of Q4 is fuckin buggy and has no support for mods
